Required documents are,
- Affidavit from Officer of Grade A or B of the State / Central Govt.
- Missing document certificate
- Processing fees of Rs.500/- and Search fee*
- Passport size photo
- Self-addressed envelope with Stamp (Rs.60/- can be affixed) for despatch of the document to the applicant
The Transcript card issued will be a Duplicate Transcript Card.
